Sacramental Record Request

(please read before submitting form)

• A baptismal certificate contains the information for all of the sacraments you have received.
• If you only received First Communion and/or Confirmation at St. Rose of Lima, the church at which you were baptized received that information from St. Rose of Lima back when you received those sacraments. Therefore, you will actually need to request a copy of your baptismal certificate from your church of Baptism and they will put the information for First Communion and Confirmation on the back of the baptismal certificate even though you received those sacraments at St. Rose of Lima. (This may seem confusing but it is the official way the Catholic Church does sacramental records. If you receive your baptismal certificate and this information is missing, please contact the St. Rose of Lima church office.)
